There are no example of this form shown in modules T2 and T12 at present. An example of the form can be found in GE/RT8001, Changes to Specified Operations Publications as amendment number 12/06s and 13/06f. |

Reporting a signal/AWS/TPWS/ATP/TVM failure or irregularity Form RT3185 is used by drivers and signallers to collate information when reporting a failure or irregularity of a signal, AWS, TPWS, ATP or TVM. The form has been amended to include updated references following the reissue of module TW5 - Preparation and movement of trains: Defective or isolated vehicles and on-train equipment. Other than the references there are no other changes to the wording on the form. An example of the form is also duplicated in Rule Book module S3 - Train warning systems (AWS and TPWS) and reporting signalling failures and irregularities. |
